Pop-Up menu (Log Entries)

First, I apologize if this topic was addressed already. Through a search and looking through the Forums, I could not find this situation.
We have installed the custom report bundle and love the features. One issue stymies me and I believe that it may have a simple solution.
Under the Log Reports/Student Discipline Reports, under Consequence, the number shows instead of the name (Expulsion, Suspension, etc). I have played around with the Log Entry Setup screen at the District Level and have made some revisions (with zero luck.)

THE ORIGINAL (showing on report as a 1,2,3, etc.):

1;Expulsion (EMIS)
2;Out-of-School Suspension(EMIS)
3;In-School Suspension(EMIS)
4;In-School Alternative Discipline(EMIS)
5;Out-of-School Suspension - Half Day

Where in the world is it pulling the "1,2,3, etc, if I removed it?

If anyone has a solution, I would be very happy to hear of it.
